Insurance:
Currently, Dana Hockenbury, MA, CCC-SLP, COM is not an in-network provider. Therefore, she is unable to bill insurance companies directly or accept insurance of any kind, including Medicaid. The code for the evaluation is M26.59 if you’d like to check with your plan.
Schedule:
The initial evaluation runs approximately one hour. The 8 therapy sessions meet every two weeks and run 55 minutes. All materials required to participate in the therapy program are complimentarily supplied.
Payment Options:
Currently I accept payments made through Venmo, PayPal or Zelle.
Dana does not want finances to be a barrier to you benefiting from her services and achieving optimum health. For patients who cannot otherwise afford the visit, she offers a variety of options and accommodations. Please contact her for more details.
Cancelation Policy:
Cancelation or rescheduling of appointments must occur a minimum of 24 business hours in advance. Unfortunately, a fee must be charged for same day cancelations, changes or ‘no shows’.
